When I first started researching my Smiths and Fakes, I didn't know who was kin to who (still don't for most of them LOL) but for some reason I suspected Odes Fakes was somehow related and I got a copy of his SS Application. If either of you don't have it and want it, let me know and I'll send it on. I can either give it on paper or scan it, whichever you prefer.
